3. Sign and return CSP's Antitrust Guidelines.

4. Pay annual dues (see payment schedule below):

• Chemical Industry: CMS Providers, Chemical Suppliers, Consulting Firms -- $5,000 (Year 1); $10,000 (Year 2); $15,000 (Year 3 and subsequent years)
  Members in this category are Tier 1 CMS providers, chemical manufacturers, Tier 2 suppliers, chemical distributors, waste management companies, and any type of consulting firm including management consulting, environmental consulting, environmental engineering, chemical management consulting, software development, and independent consultants

• CMS Customers and Other Companies -- $1,500
  Members in this category include manufacturers who contract for chemical management services, all other private sector companies, and insurance companies

• Academic, NGO, Government -- $150
  Members in this category include individuals affiliated with an academic institution, nonprofit organizations, trade associations, and government agencies
